We always welcome new patients and we are glad that you are here! As a new patient, it can be overwhelming trying to remember everything that you need to bring with you. Hopefully we can help make it a little easier for you!
Before your appointment, we need 5 forms to be filled out. These forms can be found on our website or on your text confirmation. If you have difficulty accessing these forms online, you can come in 5 minutes before your scheduled appointment time to fill it out in the office. These forms help us better prepare to see you and ensure your appointment is as efficient and convenient for you as possible.
In addition to these 5 forms, we would also need a copy of your insurance card (if applicable). This helps us more accurately prepare a financial estimate for you.
And finally, if you have any additional x rays, referral letters, or previous dental work information (such as implant brands, etc) you can send it to us via text (571-305-1888) or email (info@chantillydentalimplant.com).
To recap, for your first appointment you need 3 things: the five new patient forms, your insurance card/information, and any previous dental work information.
