Appointment Cancellation Policy:

During this time, it is important that we accommodate as many patients who need dental work as possible while also taking necessary and extra precautions to prevent the spread of disease. As such, we are not double-booking patients at this time, and each appointment time is solely set aside for you. Therefore, to secure this appointment time, we ask that you provide us with your credit card information over the phone (NOT TEXT) in the event that you break your appointment without notice 48 hours prior to it, which is subject to a $100 break fee. Each appointment time is valuable, so out of courtesy for our other patients, please notify us 48 hours prior to your appointment if you need to reschedule it. 

New Patient Forms:

Please click the following link and fill out all the forms online prior to your arrival. (Please read our Notice of Privacy Practice first to sign the HIPAA form). Thank you.

Insurances Accepted:

We participate in most PPO insurance plans. Even if you can’t find us from your network, come visit us anytime to check out your own benefit information. Many of our patients with out-of-network plan also receive some insurance benefit per their plans.

In-House Cleaning Plan

No insurance? No problem! Register for our In-House Cleaning Plan. It could be better than an insurance. The Plan includes:

  • Cleaning (2 times / year)
  • X-rays
  • Examination (2 times / year)
  • Fluoride varnish
